This is a patient safety reminder for Pharmacists:

Please consider the following if receiving prescriptions from these clinics.

•        Prescriptions written by Dr. Li are valid if dated prior to July 15, 2016. This would include refills that continue past July 15, 2016 as long as the original prescription was written prior to that date while his license was active.

•        No other prescribers from these clinics have been suspended as of August 11; therefore, their prescriptions are still valid.

•        If you question the validity of a prescription from these clinics, please contact the insurance plan or Pharmacy Commission for clarification before rejecting the prescription. The Pharmacy commission number is 360-236-4861.

•        License status can always be checked on the Department of Health website at and Provider Credential Search.

The attached documents have some resources and referrals to help direct patients to other care if needed.

•         If you have a specific Pharmacy Commission questions contact or 360-236-4861.

•        If you have general concerns or want to let the Department of Health know about how SPC closure is impacting your operations you can email them at ProviderInfo@DOH.WA.GOV
