7:45-9:00: (1.25 hours of PQAC CE) Presentation by ASCP National President Joe Marek, R.Ph., CGP, FASCP

Transitions of Care and the Changing Landscape of Consultant Pharmacy

At the completion of this program, the pharmacist and technician will be able to:

Discuss medication-related problems associated with transitions in care settings

Identify areas of opportunity for pharmacists to employ the Pharmacists Patient Care Process in transitions of care

Describe the current state of the consultant pharmacy practice and the regulatory changes that are shaping its future

Discuss comprehensive medication management, collaborative practice agreements and the integration of the consultant pharmacist into the interdisciplinary team


This CE is PQAC accredited and to receive credit you must turn in a completed evaluation with the secret code which is given at the end of the activity. WSPA will email a statement of credit within 4-6 weeks of the event. This will be your official proof of attendance.
